1095-C

Due to the Paperwork Reduction Act of 2024, employers are no longer required to furnish Forms 1095-B and 1095-C to individuals unless requested. Beginning January 1, 2026, if you want a copy of your 1095-C and have not opted into electronic delivery, you will need to email Amanda White, at awhite@bradleyschools.org, or call 423-476-0620, ext. 2007. You can also mail a written request to the address below:
